Take out the inserts, hose everything down with a garden hose, and let air dry.  You could also try some Febreeze with the bacteria killer crap in it after they dry.

Occasionally you can throw them in the wash with some towels - for you married folks - do it when shes at work.  Otherwise when they are wet brush them off good and stuff them with newspaper and dryer sheets.  You might have to change it once or twice before they completely dry.

Wash them in soap and water and dry in direct sunlight :)

Usually wash mine in the washing machine.  Remove and let air dry.  They're smelling April fresh in no time. ;D

Toss pad to washer after each ride. Spray inside of shoes with anti-bacterial and sun dry. If not enough heat outside I just usually, as what mksandoz said, stuff crumpled newspaper inside my shoes.
But it's always good to have at least two pairs so you will not be riding with a wet shoes.

The only thing that you can do to remove the smell of you shoes is to soak with detergent. After that, you rinse it thoroughly and expose it to the sunlight. Just to make sure if you wear it again you apply a foot care product in your feet.
